Porcelain Veneer Insurance Policy

We assure our patients that we will repair or replace any crown or veneers for any reason for 5 year provided that:

• repair or replacement is performed in our office
• tooth/implant can be saved
• routine cleanings and examination (every 6 months) in our office
• immediate alert to the office of any perceived problem or discovered at the 6 month cleaning

If for any reason additional treatment is needed at the time of repair or replacement ( such as decay removal, root canal, extraction or implant placement), an additional fee will be charged.
